  DO I NEED TO DISCLOSE MY HOME ADDRESS ON THE COVER PAGE?



No. Since the Form 700 is a document available for public review, you may use your business/office address.


  DO I NEED TO DISCLOSE MY INTEREST IN MY PRIMARY RESIDENCE OR INVESTMENTS IN DIVERSIFIED MUTUAL FUNDS?



No. Your primary residence and mutual funds registered with the Securities and Exchange Commission (SEC) are not reportable.

  ARE THERE OTHER INVESTMENTS WHICH I AM NOT REQUIRED TO DISCLOSE?

Yes. You are not required to disclose bank accounts, savings accounts, and money market accounts; insurance policies; annuities; shares in a credit union; government bonds; retirement accounts invested in non-reportable interests such as insurance policies, diversified mutual funds or government bonds.

  IF MY SPOUSE OR REGISTERED DOMESTIC PARTNER OWNS HIS OR HER OWN BUSINESS, WHERE MUST I REPORT THIS INTEREST?

You must report on Schedule A-2 your community property interest in your spouse’s or registered domestic partner’s share of the gross income to the business entity.

 

  WHAT IS AN INTEREST IN A REAL PROPERTY?

An interest in a real property can be an ownership interest; a deed of trust, a lease, easement, or option to acquire property; or your spouse’s or registered domestic partner’s interests in real property that are legally held separately by him or her.

  DO I NEED TO DISCLOSE MY MORTGAGE AND CREDIT CARD LOANS?

No. You are not required to report loans from commercial lending institutions if the loan is made in the lender’s regular course of business and on the same terms available to other members of the public.

  DO I NEED TO REPORT BENEFITS FROM OTHER GOVERMENT AGENCIES?



No. You are not required to report income or benefits received by you or your spouse or registered domestic partner from a federal, state, or local government agency. This includes your own City salary.

 

  WHAT IS THE GIFT LIMIT THIS YEAR?



Assuming this is a gift that you can accept under City law, the State gift limit is $390.

 
  DO I NEED TO REPORT ALL GIFTS THAT I RECEIVED OR THAT MY FAMILY RECEIVED?



You must report all gifts over $50 (Schedule D) regardless of the location of the donor. (Look to the directions on the Form 700, for exceptions.) In addition, the City ordinance requires that you file a Family Gift Reporting Form for gifts received by your spouse, domestic partner, and dependent child if the gift would have been prohibited had it been given to you. This form is filed with your Form 700.

  WHAT IF I LEARN OF NEW INFORMATION OR MADE A MISTAKE IN THE FILING?



You may amend the schedule that needs to be revised at any time. Amendment schedules can be obtained from the Office of the City Clerk or at www.fppc.ca.gov.



  ARE THE COMPLETED FORMS PUBLIC DOCUMENTS?


Yes. Statements of Economic Interests are public documents. The City Clerk’s office allows any member of the public to inspect and copy any statement.

  • Forms are available for public inspection during the agency's regular business hours.
  • No conditions may be placed on persons seeking access to the forms.
  • No information or identification may be requested from persons seeking access.
  • Reproduction fees of no more than 10 cents per page may be charged.

  WHY DO I NEED TO FILE?


The Political Reform Act requires most state and local government officials and employees to publicly disclose their personal assets and income. They must also disqualify themselves from participating in decisions that may affect their personal financial interests. The Fair Political Practices Commission (FPPC) is the State agency responsible for issuing the Statement of Economic Interest (Form 700), and for interpreting the law’s provisions.
