What is a L.I.F.E. File?
A Lifesaving Information For Emergencies (L.I.F.E.) File is a magnetic pouch that stores your medical information on your refrigerator. It’s a simple, yet critical, tool that can help emergency responders quickly learn important medical information about you in a time of crisis, such as your past medical history, current medications, and allergies. Everyone benefits from having a L.I.F.E. File, but it’s highly recommended for individuals who are more likely to call 9-1-1, such as older adults or those with a chronic disease or illness.
How to put Together a L.I.F.E. File
Putting together a L.I.F.E. File is easy and can make a big difference during a medical emergency. It’s as simple as four easy steps:
- Complete the form in English. If English isn’t your primary language, list a reliable translator in the “Additional Information” section that emergency responders can call in order to help answer questions. If you need help filling out the form, speak with your doctor.
- Fold the form and place it in your red or blue magnetic pouch.
- Place copies of advance directives* in the pouch. Examples of advance directives include a physician orders for life-sustaining treatment, do-not-resuscitate order, and living will. If any such documents are placed in the pouch, mark “Yes” under the “Advanced Directive” question.
- Stick the pouch to your refrigerator. If the pouch doesn't stick to your refrigerator, place the pouch in a ziplock bag and tape it somewhere visible on your refrigerator. Lastly, be sure to keep this information up-to-date and accurate.
