How to Request a Public Record
Fire Report or Incident Report
A fire report or incident report contains information about SJFD's response to a specific call. These reports include details on, but are not limited to, response times, responding units, nature of the call, or actions taken.
The fastest way to obtain individual fire reports or incident reports is by submitting a request through the City of San José Public Records Center online portal. Please be sure to include your contact information, including your email and phone number, in case we need to clarify your request or the location to which you would like the report sent.
Alternatively, these records can be obtained by sending the request by mail to:

Please note: SJFD does not charge for services; therefore, we do not have any medical bills to provide. For ambulance and billing records, please check with the responding ambulance company directly.
To locate the records you're requesting, please include the following information:
- Business name (if it's a commercial property)
- Type of Incident (Fire, EMS call, Vehicle accident, etc.)
- Date of the incident
- Time of the incident (if it's within your possession)
- Incident number (if it's within your possession)
- Location of the incident (address and/or street)
Pre-Hospital Care Report
An SJFD Pre-Hospital Care Report (PCR) contains information regarding the medical treatment of an individual patient by the San José Fire Department.
The release of medical records is restricted by the terms of the Health Insurance Portability and Accountability Act (HIPAA); however, individuals may request a copy of their own patient care record. These records may also be obtained by a parent, guardian, conservator, those who have medical decision-making authority for an individual, agencies having a patient-signed consent to release medical information, or by the executor of a decedent’s will.
SJFD Pre-Hospital Care Reports cannot be requested by phone, email or through our records portal, as proof of identity is required to establish eligibility to receive these records under the HIPAA.
To request copies of SJFD Pre-Hospital Care Reports, please mail the following to:

- SJFD HIPAA Release Form
- $15 Check (made out to the San José Fire Department)
- Copy of Patient's Photo ID
- Incident Information (Incident Number, Type, Date, Location, and Time).
Property Reports
The SJFD Bureau of Fire Prevention is responsible for a number of fire-related property reports, including fire inspection records and hazardous materials information. These records may be requested by mail, phone or email. Contact information is provided below.

Please note that the SJFD Bureau of Fire Prevention only maintains fire-related records.
