San Jose Implementation Plan for Community Plan to End Homelessness

The City of San Jose is committed to taking concrete action steps to drive implementation of the Community Plan to End Homelessness.  The Implementation Plan builds from the three core strategies of the Community Plan and includes all City Departments in a One Team approach to addressing homelessness in the city.

2020-2025 Community Plan to End Homelessness

The 2020-2025 Community Plan to End Homelessness is a product of community engagement led by the Santa Clara County Continuum of Care. It will serve as a roadmap for ending homelessness in Santa Clara County and is organized around three core strategies:

  1. Addressing the root causes of homelessness through system and policy change
  2. Expanding homelessness prevention and housing programs to meet the need
  3. Improving quality of life for unsheltered individuals and creating healthy neighborhoods for all
