This form specifies the benefits you provide to workers. You are required to submit verification documents to receive credit for any benefits you provide.

This form specifies the workers you plan to use on a project. The work classification assigned to a worker must match the classifications on the DIR website.

Certification of Posting and Distribution (Prime Only)

California Labor Code requires the current copy of the General Prevailing Wage Determination must be posted at a public works construction job site. Use this form to confirm that the current General Prevailing Wage Determination has been distributed and posted.

This form will allow you to identify each subcontractor and their duties.

Wage Determination Packets

The packets below provide wage rates and predetermined increases for DIR craft classifications. The packets also include information regarding craft classifications that are not allowed on City public works projects.
